Option 2. Offline Alternative 

1. Secure Application for Admission form from the ESSU Campus nearest to you or from the information desks of Municipal Halls within the province.
2. Fill out the University Admission Application Form (ESSU-ACAD-100a) completely.
3. Attach the photo copy of your Report Card(DepEd Form 138e) / Transcript of Records.
4. Put the form and your Record Card / Transcript of Records on a brown envelop and seal it.
5. Drop your documents on designated drop box.
