Option 2. Offline Alternative
- Secure Application for Admission form from the ESSU Campus nearest to you or from the information desks of Municipal Halls within the province.
- Fill out the University Admission Application Form (ESSU-ACAD-100a) completely.
- Attach the photo copy of your Report Card(DepEd Form 138e) / Transcript of Records.
- Put the form and your Record Card / Transcript of Records on a brown envelop and seal it.
- Drop your documents on designated drop box.
